15 June 2026 – Following the discovery that Instagram votes had been omitted from the interim Employee of the Week calculations, Harley was formally advised by the Department for Elections & Governance that, as Head of Department, ultimate responsibility for departmental outputs rested with her, irrespective of her personal involvement.

An internal memorandum further noted that her recent absence from departmental duties had not been supported by the required Form HR-17b (Exception from Weekend Overtime Duties).

Harley was instructed to: oversee a full recalculation of the election results; prepare and issue a public statement acknowledging the omission; and submit Form HR-34c (Exception for Omission of Exception Documentation).

These steps were completed and no further action was taken.
