Subject: Employee of the Week Election Review

Following a review of polling data submitted by the Department for Human Behaviour & Statistical Analysis, a discrepancy has been identified between the reported vote totals and the number of votes received.

Subsequent investigation has established that votes submitted via Instagram were omitted from the recent calculations prepared by Buzz (Senior Human Behaviour Analyst) and Toffee (Junior Data Acquisition Officer) during your absence.

The Department for Elections & Governance accepts that the omission was not intentional. However, as Head of Department, ultimate responsibility for departmental outputs rests with you, regardless of whether or not you were personally present during their preparation.

The Department further notes that your recent absence from departmental duties was not supported by the appropriate documentation. Specifically, Form HR-17b (“Exception from Weekend Overtime Duties”) was not submitted.

Accordingly, you are instructed to:

• Oversee a full recalculation of election results.

• Prepare and issue a public statement acknowledging the error.

• Submit form HR-34c (“Exception for Omission of Exception Documentation”) forthwith.

The Department expects the public statement to be delivered at the earliest reasonable opportunity. Failure to comply may result in further disciplinary action.

The matter is considered ongoing.
